We may have to request further accompanying documentation after in order to complete your petition to the courts, but this is the ordinary course of preparing your application. In order to start any Name Change Application we will also need to be provided a copy of your birth certificate and will eventually need a certified copy or original. If it is from outside of the United States, it will need to be accompanied by certified translation and it is usually best to have the translation "certified" by the appropriate consulate via the Hague Convention.
